Payment is due at time of service. We accept cash or check. A service fee will be charged for any credit card charges.
We do not accept insurance but can provide a superbill for insurance reimbursement upon request.
Broken appointments will be charged a half fee upon the first offence and a full fee on any after.
To avoid a broken appointment fee please call to cancel or rescheduled your appointment within 24 hours. Appointments canceled in less than 24 hours will be charged a broken appointment fee.
If you wish to submit your claim to insurance you will be able to download a Superbill, complete with your cousnelor's license inforamtion, National Provider ID, Tax ID, billing codes, signature, and payment. You will need to submit this to your insurance for reinbursment for services.
Virtual appointments will take place through the client portal. Make sure you have a strong internet connection. The portal will work on your computer, not on a phone or tablet. Make sure your computer is up to date and that Adobe is up to date as well.
